GIR = (rate · dextrose %) / (weight · 6) mg/kg/min.

Compute the glucose infusion rate in mg/kg/min from the IV fluid rate (mL/h), dextrose concentration (% w/v), and patient weight (kg). The factor 6 = 60 (min/h) ÷ 10 (g/dL → mg/mL). Neonates typically receive 4-8 mg/kg/min.
